An Affiliate membership application is the only item required to process an application.
OSNM reserves the right to screen all applications for membership to assure the best interests of the members it represents are met.

Application Procedures

  • Download and complete the Affiliate Membership application below and return to OSNM by mail or by fax.
  • The membership representative will review the applications.
  • A letter of acceptance will be issued upon acceptance of the application.
  • Upon receipt of your Affiliate Membership fees, a Certificate with appropriate seal will be forwarded to you.

  • Published Qindex study highlights quality in LTC homes

    The Association is pleased to announce that last year’s Qindex research conducted in partnership with the University of Toronto has now been published in Longwoods’ Healthcare Quarterly.

    The Qindex is a composite metric of the nine publicly reported CIHI quality indicators (falls, antipsychotic use, pain, worsened pain, restraint use, worsened physical functioning, improved physical functioning, depression, and pressure ulcers).
